Preoperative ATS affects ACL reconstruction outcomes. We assessed 100 ACL patients using supine lateral X-rays. Significant differences were found in ATS and tibial plateau angles between injured and uninjured knees. Patients needing medial meniscus treatment showed notable differences in ATS and RPIR. RPIR could be a reliable measure. Understanding ATS preoperatively could improve outcomes. ATS, space of ACL, and RPIR are useful indices of tibial anterior subluxation.
