This study aimed to determine the optimal reference points for the target posterior tibial slope (PTS) in robot-assisted total knee arthroplasty (TKA) by investigating the difference between the PTS targeted during surgery and the postoperatively achieved one. In the present study, we investigated the differences between the target PTS and postoperative one on three-dimensional computed tomography images, and the factors that influence the proximal tibial reference point and postoperative PTS, based on analysis of 62 knees that underwent primary image-free robotic-assisted TKA. We found that, in patients with smaller or larger preoperative PTS, the proximal tibial reference point should be placed posterior or anterior to the anterior edge of the anterior cruciate ligament footprint in image-free robotic-assisted TKA.
