RISK FACTORS OF VARUS PROGRESSION AFTER ARTHROSCOPIC REPAIR OF MEDIAL

Description

Purpose To investigate the risk factor of varus progression after arthroscopic medial meniscus posterior root tear (MMPRT) repair and to compare the clinical outcomes of arthroscopic repair of MMPRT according to varus progression.

Methods Patients who underwent isolated arthroscopic repair of MMPRT between January 2015 and December 2020 at one institution and had a follow-up of 2 years were included in this study. Participants were categorized into two groups: less than 5.9% increase of weight bearing line ratio (Group A), more than 5.9% increase of weight bearing line ratio (Group B). Various factors, including demographic features and radiologic findings, were analyzed and compared between the two groups. Meniscal healing, medial meniscal extrusion (MME), articular cartilage grade were assessed at coronal magnetic resonance imaging preoperatively and 1 year postoperatively.

Results The final cohort consisted of 34 patients in group A and 46 in group B. Preoperative weight bearing line ratio, cartilage lesion of medial compartment were not different between the groups. Preoperative medial meniscal extrusion were statistically less in group A than group B (2.58 ± 0.6 vs 3.53 ± 0.7, p = 0.048). Patient-reported outcomes at 2-year follow did not different between the two groups. In a logistic analysis, odds ratio of MME was 2.1 (P = 0.003) and the cut-off values of MME was 3.02mm.

Conclusion More preoperative MME is risk factor of more varus progression. Even in the group with more varus progression, there were no differences in patient-reported outcomes at 2-year follow-up.
