SAVE THE MENISCUS - MODIFIED HENNING OPERATION WITH COLLAGEN MEMBRANE

Description

Our modified Henning technique is based on three pillars:

1.stable suturing of the meniscus,

2.placing a graft from the iliotibial band on the meniscus

3.covering it with a collagen membrane.

Suturing the meniscus provides initial stabilization, the iliotibial band graft provides a scaffold for the reconstructed tissue, and the collagen membrane provides biological stimulation of healing.

Methods: A prospective study was planned, taking into account the results of 10 patients. All patients underwent the same surgery, performed by the same team, to ensure reproducibility.

Pre- and post-operative assessment was performed using the International Knee Documentation Committee (IKDC) questionnaire, the Lysholm scale and the Knee Injury and Osteoarthritis Outcome Scale (KOOS). Statistical analysis of the results was performed using

Shapiro-Wilk tests and t-tests for dependent variables.

Pre- and postoperative magnetic resonance imaging of the knee was also analyzed. The height of the meniscus - posterior horn was measured on coronal and sagittal scans, the size of extrusion and the WORMS scale for menisci were measured on mr scans.

 

Results: A significant increase in the values of the results in the postoperative assessments of the above-mentioned scales was observed. The average IKDC scale score increased from 50.57% to 75.4% (p-value - 0.004); the average score of the Lysholm scale was 56.8% out of 90% (p-value - 0.0002), and the average score of the KOOS scale from 53.5% was 86.2% (p-value - 0.0001).

 

The average height of the meniscus before the operation was 2.2 mm, but after the operation it increased to 6.1 mm and remained until the control examination on 12 months.

 

The average preoperative extrusion value was 3 mm and remained at this level until 12 months after.

 

The mean WORMS score for meniscus was 4.3 in the preoperative assessment, and in the postoperative assessment it was 3.1 after 12 months.
